The individuals living at Centerville enjoy private bedrooms personalized to their preferences, a sensory room and spacious living areas. At Centerville, individuals are strongly encouraged to participate in daily programs promoting independence in all areas of living, including personal cares, dining preparation, communication skills, community and in-house activities. Centerville provides 24-hour awake staff, 24-hour nurse consultation, as well as a consultant dietician.
Lydia

Lydia house is a beautiful four bedroom (waiver) home located in Maplewood. Lydia has two bathrooms, central air, a big finished basement with a laundry room and lots of storage, a beautiful front and back yard, one car garage and a shed for storage. Maplewood Mall is a short walk from our front door. We have a huge kitchen with lots and lots of cabinet space. The back deck is beautiful with an area to picnic. Lydia provides 24-hour awake staff and 24-hour nurse consultation.
Suburban

Suburban house is conveniently located off I94 in East St. Paul. It is a four bed waiver site. Suburban provides 24 hour awake staff and 24 hour nurse consultation. The individuals living at Suburban enjoy private bedrooms decorated in their choice of colors and décor and a large fenced in backyard. Our goal is to assist the individuals to be as independent as they can be through daily programs and getting out into the community.

The Douglas home has a fully accessible, single story floor plan with private bedrooms for all individuals which can be decorated to meet the resident’s preference. From the front patio residents are able to view the train yard and also have a courtyard off to the side as well. Douglas provides person-centered planning and a variety of community opportunities to enrich the lives of the residents who live there. Family involvement is an important part of our residents’ quality of life and is greatly welcomed and encouraged. The home is a single story fully accessible space with private bedrooms that can be decorated to meet the preferences of the individual. Foss

Foss House is a 6-bed ICF/DD home located in St. Paul. The home has private bedrooms which can be decorated to an individual’s preference as well as an open, fully accessible floor plan. At Foss individuals are encouraged to participate be as independent as they are able through person-centered programming. Community Integration, Family Involvement and a host of in home activities assist in personal growth and quality of life for the individuals who live at Foss. Drexel Court

The home has an open backyard which leads to a city park. At Drexel, individuals have the opportunity to participate in community integration and person-centered planning like in all homes. Family and Community involvement is greatly encouraged. Drexel has 24 hour awake staff and nursing consultation.
Chaparral

Chaparral is a 4 bed waiver home located in Apple Valley. The home has 2 stories with 3 bedrooms on the main floor and 1 bedroom in the lower level. The stairway to the lower level has a seated lift to increase accessibility. Additionally the home has a large deck off the second story of the house and also has a cement pad in back for playing basketball. At Chaparral, individuals have opportunity to participate in person-centered-planning and enjoy a variety of opportunities in the community based on the residents’ preference. The home has 24 hour awake supervision and nursing consultation. Family and community involvement is welcomed and recognized as a valuable part of our residents’ lives.
