Community Support Specialist

Phoenix Service Corporation (PSC), sister corporation to The Phoenix Residence, Inc., has an exciting opportunity for someone who would like to make a difference in the community! PSC is now accepting applications for a part-time Community Support Specialist (CSS). Please note this position is part-time, but has the potential to become full-time.

Job Description:

The Community Support Specialist will work directly with individuals served by PSC in all aspects of life. They will find and maintain housing, and promote independent living by decreasing dependence on caregivers.

This position requires cultural competence and the knowledge of state and county programs and services for housing stabilization. The CSS works independently and in collaboration with others in helping to facilitate housing.

The CSS is the liaison between clients and outside agencies, including vocational, rehabilitative, cultural, recreational and medical services. The CSS advocates for the individuals and their needs, conducts assessments, coordinates ILS program plans, and meets weekly with the individuals. The CSS is also responsible for completing and maintaining necessary documentation for CSS services. They may teach individuals how to budget and manage money, access community resources, coordinate medical care, or maintain their home.

PSC is dedicated to providing the necessary supports and services to help adults remain independent and active in their own community. Our goal is to provide the services most appropriate to the needs and desires of the individual—we focus on what is important to the person.

Don’t have a vehicle?  No problem! PSC has multiple vehicles available for our CSS staff. The office is conveniently located at 706 S. Robert Street in St. Paul, only one block from the metro bus line. The CSS position is flexible with no weekends, evenings or holiday hours.

Job Requirements:

  • Previous related field experience preferred, such as assisting people with disabilities and/or in the Health and Human Services field
  • Experience with person-centered planning approaches
  • Knowledge of disabilities and/or chronic illnesses
  • Knowledge of independent living skills management and ability to coordinate activities and services
  • Must be able to work independently without direction or supervision
  • Ability to read, write and speak the English language
  • Ability to travel among agencies, hold a valid driver’s license, and maintain a safe driving record
  • Excellent communication skills, both written and verbal, and possess exceptional listening skills
  • Ability to work with diverse clients
  • Ability to work flexible hours
